Paver designations are for military veterans only. The qualifications to verify military status, the following resources are available:

Note:
Paver orders will be accepted on an annual basis, with the deadline to order on July 31st of each calendar year. They will then be placed in the ground of the designated pathway of the Huber Heights Veterans Memorial at Thomas A. Memorial Cloud Park. They will be installed by the following Veterans Day of the same calendar year or as resources allow. All paver placements and final accepted wording will be at the discretion of the city administration. For conformity, the city will purchase and install all pavers.
Replacement of a damaged paver will be the responsibility of the purchaser. If a brick becomes severely damaged and the purchaser cannot be located the paver may be removed.
Personalized engraved or etching will be covered in the fee of the brick which will be $125.00 per brick. This figure may be adjusted to cover any costs. The engraving/etching shall include veterans name, service, and dates only. Any exception to this would be at the discretion of the city administration.
The cost per paver is $125 for the paver and installation. Once you have completed the form and hit submit you will be taken to the payment screen.
To qualify for a paver, honorees must meet the following requirements:
Active-duty military personnel of the United States Armed Forces; AND
Current Huber Heights resident or Huber Heights is an individual's hometown, OR
An immediate family member of a Huber Heights resident (parent, spouse, child, sibling, grandchild, child-in-law). OR
Past resident, graduate, or community leader.
As part of the application, you MUST upload:
Verification of military status (Military ID or DD-214 form)
